Answers before the call
The questions we hear most from physician groups considering an in-office lab.
PlugPOL’s program is built around the In-Office Ancillary Services Exception (IOASE) to the Stark Law (42 C.F.R. §411.355(b)). Your practice group holds the CLIA certificate, bills payers directly under your own NPI, and provides laboratory services to your own established patients on-site. PlugPOL provides practice management and operational services only, not the laboratory services themselves. All agreements are structured accordingly and reviewed with your healthcare counsel before execution.
PlugPOL handles staff recruitment and onboarding. Lab professionals work under the supervision of your licensed physician director, consistent with CLIA personnel requirements. Your practice maintains the supervisory relationship required for IOASE compliance. PlugPOL manages the operational side.
Your existing payer contracts remain with your practice group. Laboratory services provided to your established patients are billed under your group’s existing NPI and payer enrollment. PlugPOL assists with adding laboratory CPT codes to your existing credentialing where needed. The payer relationship stays entirely with your practice.
There is no fixed minimum, but the economics are most compelling for practices seeing 200 or more patients per month who require routine laboratory work. We review your current test referral patterns in the first step to confirm fit, before any commitment is made.
Your practice group holds the CLIA certificate. This is a structural requirement of the IOASE. It ensures laboratory services are provided by your group, to your own patients, in your own office space. PlugPOL assists with the CLIA application and manages the operational requirements, but the certificate is yours.
For tests that can be run in your office, your providers order and process those on-site rather than sending them out. Specialized tests requiring reference lab equipment can remain at outside labs. We map your current referral patterns during the assessment to identify which tests are best suited for in-office processing and where the revenue opportunity is greatest.
